Midland PPP Customer Information

This page contains helpful information to assist our PPP customers with loan forgiveness applications and pending approval information. The information below is only relevant for customers who received their Paycheck Protection Program loan through Midland States bank. 

Important Note: This page is provided as a customer courtesy. The SBA is responsible for the rules of this program and determining the amount of loan forgiveness, which can include full, partial, or no forgiveness. Updated information on the terms of Paycheck Protection Program loan forgiveness is available on the U.S. Treasury and SBA websites.

Forgiveness Application Status Update

The SBA has started to send back approvals for borrowers who fully completed and submitted PPP Loan Forgiveness applications. As we receive confirmation of approvals for our borrowers, Relationship Managers will notify customers via email. Please review the information contained on this page for details related to timing on the review and pending approvals for all Forgiveness Applications.

SBA Loan Forgiveness Application Instructions for Borrowers

To apply for forgiveness of your Paycheck Protection Program (PPP) loan, you (the Borrower) must complete the SBA loan forgiveness application as directed in the SBA documentation, which is then submitted to us (the Lender for your PPP loan). Information and support items to assist with the forgiveness process are included below.

We know that it may take time for you to gather the support documentation and we suggest you start right away. Once you are eligible to apply, we’ll email you with instructions to begin the application.

The SBA recently revised the loan forgiveness documentation and has also added an EZ Program. Please review the following information to determine which program is appropriate for your business and to start preparing for your forgiveness application.

For Midland PPP customers, there is no need to fill out or submit any paper documents. These documents are provided for reference to begin gathering information for your application.  However, a Midland Relationship Manager will be contacting each PPP borrower with information to electronically complete forgiveness applications.
