Midland PPP Customer Information

This page contains helpful information to assist our PPP customers with loan forgiveness applications. The information below is only relevant for customers who received their Paycheck Protection Program loan through Midland States bank.

Important Note: This page is provided as a customer courtesy. The SBA is responsible for the rules of this program and determining the amount of loan forgiveness, which can include full, partial, or no forgiveness. Updated information on the terms of Paycheck Protection Program loan forgiveness is available on the U.S. Treasury and SBA websites.

SBA Loan Forgiveness Application Instructions for Borrowers

To apply for forgiveness of your Paycheck Protection Program (PPP) loan, you (the Borrower) must complete the SBA loan forgiveness application as directed in the SBA documentation, which is then submitted to us (the Lender for your PPP loan). Information and support items to assist with the forgiveness process are included below.

We know that it may take time for you to gather the support documentation and we suggest you start right away. Once you are eligible to apply, we’ll email you with instructions to begin the application.

Should you have questions after review of the documentation below, please contact your Relationship Manager. If you do not have a dedicated Relationship Manager or need assistance contacting a member of our team, Customer Service is available at 855-696-4352 to assist.

The SBA recently revised the loan forgiveness documentation and has also added an EZ Program. Please review the following information to determine which program is appropriate for your business and to start preparing for your forgiveness application.
